If your child is interested in participating in Mt. Blue Middle School athletics, the documentation and forms included below must be completed before he/she will be eligible to participate.
New in Fall of 2026 - Sports forms are completed through your PowerSchool Parent Account
ATHLETIC PARTICIPATION & CONSENT FORM
SPORTS PHYSICAL CLEARANCE must be on file - A typical sports clearance is between 15-24 months depending on the provider. These can be faxed to the school, 207-778-5810.
